Listed below are some things you need to be aware of regarding forms: > Some of the forms may have a defined time period to fill these out online. The link will be removed when that period closes. > For several forms, once filled out, only need to be reviewed and edited (if needed) each year. > The forms process requires you to mark each form completed as your digital signature to the content of the forms. This has to be completed each year and will be monitored by the campus. > After marking a form completed, you can open the form for editing. After saving, you will need to mark it completed and perform the Final Submission step again as well. > Only members of the 1st Family of the student can fill out forms . If you need to make changes to your family information, please contact the campus. Accessing Forms 1. Log into Family Access. 2. A notification on the main page will appear for each student who have not been marked completed. 3. You can access forms in the notification OR at any time by selecting the link on the left menu: 20xx-20xx Forms Packet . Completing the Forms NOTE:If there is information such as an address which you cannot update on a form, please contact the campus to change that information. 1. Select the link to the right for each form. The form will display on this page. NOTE: You can chose to view it in full screen at the top right or print it using the button on the top left of the form. 2. Review each field carefully and make changes. Enter information if it is missing. NOTE: Required fields will warn you if they are not completed and will highlight the missing field. 3. Scroll to the bottom of each form and select the option: "Complete Step X and move to step X" to mark the form completed. This serves as your electronic signature. NOTE: Links to outside sites must be accessed before you can mark that step completed. 4. You can choose the step to pay the fees within the forms process. If you have previously paid the fees you can mark the step completed or skip the step. It is optional and will say that it is "Skipped" when you submit the forms. 5. You must complete the last step in order to submit the entire set of forms to the campus. NOTE: You can fill out forms in any order and leave and return. The system will remember your completed steps. If you do not complete the last step, the campus will not have verification that you completed all forms! Editing Forms After Marking Them Completed Normal 0 false false false EN-US X-NONE X-NONE Select the forms packet link on the left menu and choose the student. Mark the forms as "Not Complete" Select the step and choose “Edit Step x” and make your changes. Mark the step completed using the “Mark Step x Completed” option. You will need to complete the final step again and choose "Close and Finish", otherwise the campus will show that you have NOT completed the forms. I forgot or do not know my Username or Password Submitted 4/2/2014 in FAQs Skyward provides an individual account for each parent connected to each of your students. There is no means for parents to create their own account. To request a Password reset link and username be sent to you by email: 1. Access the LISD Home Page, select the Parent Tab and click on the Skyward Family Access link. 2. On the resulting page, choose the Family Access login link at the top of the announcement area. 3. On the login page, choose:" Forgot your Login/Password? " Link. 4, The system will ask for a username or email address to send the reset link to. NOTE: This is going to send to the email address in our system for you. If that was never entered or has not been updated, you will not receive the email. 5. When you receive the email, follow the instructions and reset your password. The email will also have your username listed in it. NOTE: if more than one person shares an email address you will receive multiple emails. Students with your email address will not allow for the resetting of the password. If you do not receive the email or the link does not work as expected, please contact the campus and request a new password. At that time, have them check the email address for accuracy. NOTE: The campus will likely require proof of identity to perform this for you. They are required to secure student information to the best of their ability for your student's protection. How do I log into the system? Submitted 4/2/2014 in FAQs Skyward provides an individual account for each parent. You have two options to get your account information: IF YOUR ACCOUNT HAS YOUR CURRENT EMAIL ADDRESS, You can use the forgot login/password link located on the login page. The system will request your email address and send both the username and password reset link for you to change your password. Please check your SPAM folder for that email. Detailed directions are located in article on this FAQ: If the above method does not work (email is not found, the account is not enabled or you do not receive the reply) Campus staff can reset your password and provide your username. They will verify identity before providing this information. Login Procedure 1) Open the LISD home page ( www.lisd.net ) and select the "Skyward" Icon at the top of the page. 2) Click on the Family Access Login link. (LISD Staff are to use their LISD network account login credentials IF the campus connected your student to that account). Click the Sign In button. 4) If this is your first login, the system should prompt you to change your password to one of your choice. Note : If the district has no home email associated with your account or another member of your family on record, the system will prompt you to enter a new email, or decline to have an email address on record. If you do not find your answer there, please feel free to submit a ticket. Though you may submit a ticket at any time, our office hours are: Monday- Friday, 7:30 - 4:30. We do not provide support on Weekends or during School Holidays, but will respond to your ticket promptly upon our return to regular office hours.
